What Makes the PfHW Program Different?
The multidisciplinary team includes psychologists, physiotherapists, dietitians and nurses who understand the complexity of obesity management and are committed to providing comprehensive, evidence-based services in a respectful environment.
Will I need to be medically monitored by my family doctor to participate in the PfHW Program?
Yes. Your family doctor plays an important role in your journey throughout the PfHW Program as they remain your primary care provider. It is anticipated that your family doctor will support you by following you closely while enrolled in the Program. Please note that you must have a family physician’s referral in order to participate in the Program.
Is there a fee for this Program?
Yes, there is a cost of for all services at PfHW. Please view the on-line Information Session videos and read the Program Information Booklet on this website or contact us for more details.
Do you have any payment plans available?
We have several Program payment plans available. These are noted in the Program Information Booklet.
Will insurance cover any of the costs for the services I receive at PfHW?
Every insurance plan is different; therefore, we cannot tell you whether or not your plan will cover the cost of PfHW services. We will, however, provide you with any documentation required for you to submit a claim.
How often/when do Programs start?
Programs start throughout the year according to interest. Call our Administrative Assistant at 473-1189 for the tentative start date of our next program.
I am interested in participating in the PfHW Program. How do I enroll?
If you are interested in participating in the PfHW Program, the first step is to view the on-line Information Session video. During this session, you will be provided with a comprehensive overview of the PfHW Program. Subsequent to this session, there are a number of steps that have to be completed prior to being booked for your initial assessment by the clinical team. Please submit your Participant Availability Form and arrange for an apointment to have the Referral Form completed by your family physician. The purpose of the initial assessment is to assess your readiness for treatment and determine if you have any medical conditions that may rule out participation in the Program or that need to be closely monitored during your participation.
